This form is required on all transactions where personal nonpublic information is collected.  If you are involved in getting financing for a customer, before they even fill out a credit application, they should be given this form and asked to fill it out.  It is a 2-part carbonless form and comes 50 per pack.  An information/instruction sheet is included.
